The requests for information in this application are answered by the user in one of four ways:

   1. By selecting a 'Yes' or 'No' choice button.
   2. By checking a checkbox.
   3. By selecting a value from a drop-down list.
   4. By entering text in a textbox.

While the use of methods 1 through 3 is self-evident, data entry in a textbox has some restrictions:

   1. Text may be entered using characters from the following set:
          A-Z, a-z, 0-9, spaces, and the following punctuation and symbols: ),@'/:-"_.$(

   2. While the period is an allowed character, 2 or more consecutive periods will be reduced to
      1 period with one exception: If 3 consecutive periods are bound by spaces, they will be
      converted to an ellipsis symbol (…).

   3. An ampersand (&) may be entered, but will be converted to ' and '.

   4. In textboxes having more than 1 line, the newline character is allowed, and may be entered
      (using the 'Enter' key on most keyboards) to begin a new line within the textbox.

   5. Tab characters that are pasted into a textbox will be retained, but a tab cannot be directly
      entered into a textbox.  Pressing the 'Tab' key while in a textbox will cause the cursor to
      advance to the next active field in the web page.  If you wish to insert a tab character into
      a textbox, it must be pasted in.

   6. Text may be pasted into a textbox, and is subjected to restrictions 1-5.  Additionally,
      bulleted lists up to 2 levels deep will be handled.  Numbered lists will be allowed, but
      automatic indentation is not provided, and manual intervention will be required.

When you exit a textbox by tabbing out or clicking on another field, all restricted characters will be
removed from the text, and any handled formatting will occur.  Following the adjustments, the cursor
will be positioned at the next active field in the web page.

Required fields are marked with an asterisk (*) next to the field.  If the 'Submit Applications' button
is clicked while a required field or selection has not been provided, the word 'Required' will be appended
to the asterisk (*), and the cursor will be moved to the first 'empty' required field on the page.

To attach documents to your application, use the 'Attachments' box at the bottom of this page.  The box
contains the instructions for attaching documents.  Once documents have been attached, they may be viewed
by selecting the document name from the drop-down list of attachments, and clicking the 'View Attachment'
link.  Attachments may be removed by selecting the document name from the drop-down list of attachments,
and clicking the 'Remove Attachment' button.
